Adult chaperones (age 21 and older) and any "Junior Chaperone," age 18 (and out of high school) to 20, must have a background check run through the Diocese of La Crosse Safe Environment Office and complete the Diocese of La Crosse Safe Environment Training. Please indicate the dates your background check was approved, and the date you completed safe environment training in the fields below. If you do not know the dates, you can get them from your parish/school office.
